By actively ticking the safeguarding agreement during booking, you confirm that:
The agreement covers health and care information entered during booking, including allergies, medical conditions, medication or equipment, dietary requirements, access needs, emergency instructions and other safeguarding or welfare details you choose to provide.
What it is not used for
This agreement does not provide marketing permission. Marketing choices are separate and optional. It also does not replace the separate confirmation that you have authority to make the booking, or acceptance of the Terms of Service.
Record of your agreement
Elevate Fitness Site keeps an audit record showing who made the booking, the date and time of approval, how approval was given, this agreement version, and the exact agreement wording shown at that time.
Withdrawing your agreement
You may withdraw at any time through the booking in your customer portal, or by contacting Elevate Fitness Site. Withdrawal does not change the lawfulness of processing that took place before withdrawal. The approval and withdrawal audit record is retained, but processing based on the withdrawn agreement stops and booking-created care information is removed from operational booking and care-profile views.
Withdrawal does not automatically cancel the booking. However, if Elevate Fitness Site cannot safely support the child without current care information, the club may need updated information or may need to discuss whether participation can continue safely. Information may still be handled where another legal duty or emergency legal basis applies, as explained in the Privacy Policy.
Privacy information
I confirm that the medical, allergy, medication, dietary, access and emergency information provided for each child is complete and accurate, and I consent to the club using this information to safeguard the child and respond to emergencies, as explained in the Privacy Policy.
